Mishaal attended the Al Mubarakiya School in Kuwait, and then graduated from the Hendon Police College in the United Kingdom in 1960. He then commenced employment with the Kuwaiti Ministry of Interior. After his half-brother Sabah ascended to the throne in 2006, Mishaal was recognised as one of the top three decision-makers within the Al-Sabah royal family. Allegedly, he deliberately avoided assuming more prominent roles during his tenure to steer clear of political controversies and preserve family connections. In 2021, his brother, Emir Nawaf, appointed Mishaal acting emir due to poor health. As crown prince, Mishaal announced the dissolution of the Kuwait National Assembly in April 2023, citing legislative provisions granting the emir such authority in response to political unrest and deadlock in Kuwait. He represented Kuwait at notable international events, including the state funeral of Queen Elizabeth II at Westminster Abbey in London in 2022, and the wedding of Crown Prince Hussein of Jordan in 2023. He was appointed emir of Kuwait after the death of his half-brother, Nawaf, in December 2023.

Son of Ahmad Al-Sabah, Emir of Kuwait, and Mariam Al-Huwailah. He married firstly Nuria Al-Sabah in 1963, and secondly Sheikha Al-Andalus in 1990. He has issue.
